Check. Change. Control. is a 4-month program designed for those newly diagnosed with hypertension, or those who have uncontrolled hypertension. It focuses on increasing physical activity, healthier eating, and better management of high blood pressure through education and tracking, guided by volunteer health mentors who encourage participants to check their blood pressure readings.
